How to Send Photos and Medical Records to Dr. Revis

If you have medical records or laboratory tests to send to Dr. Revis, please fax them to our office. If you wish to email photos, please do so in JPG/JPEG format. There is no need to downsize the dimensions of the photos unless they are exceptionally large.

Alternatively, you may mail your records and/or photos.
